in accordance with the Law of the Republic of Belarus dated 07.05.2021 N 99-\u0417 \u201cOn the Protection of Personal Data\u201d<\/strong><\/p>\n\n\n\n

By putting a check mark in the appropriate field on the website of Fagus LLC https:\/\/fagus.by\/en\/<\/a>, I voluntarily give Fagus LLC (hereinafter – the Controller) consent to the processing of my personal data, namely: surname, name, patronymic; telephone number; email address; name of the employing organization; position in the employing organization; data that is automatically transferred by the website https:\/\/fagus.by\/en\/<\/a> during its use with the help of software installed on the User\u2019s device, including the IP address, cookie information, information about the User\u2019s browser (or other program used to access the Website), access time, address of the requested page – in any way, including, but not limited to, consent to the collection, registration, accumulation, storage, adaptation, modification, renewal, use, provision, depersonalization, blocking and deletion of my personal data.<\/p>\n\n\n\n

I am informed that the processing of my personal data is carried out for the purposes of: receiving requests for information from Users of the website https:\/\/fagus.by\/en\/<\/a> via the feedback form; processing applications for the performance of work (provision of services) by Fagus LLC; sending responses to Users to applications, requests received through clickable forms of interaction; conducting statistical and\/or evaluation studies of the effectiveness of Fagus LLC and the website https:\/\/fagus.by\/en\/<\/a> .<\/p>\n\n\n\n

I have been informed that the Personal Data Controller is Fagus LLC, located at: Republic of Belarus, 220099, Minsk, 55 Kiseleva st., off. 34.<\/p>\n\n\n\n

I have been informed that when processing my personal data, the Controller takes all necessary organizational and technical measures to protect personal data from unauthorized or accidental access, deletion, distortion, blocking, copying, distribution of personal data, as well as other illegal actions.<\/p>\n\n\n\n

The Controller processes personal data using methods that ensure the protection of such data, with the exception of the following cases: (1) in case of anonymization of personal data; (2) in case of public availability of personal data; and in compliance with the established requirements for ensuring the security of personal data, requirements for technologies for storing such data in accordance with the current legislation of the Republic of Belarus.<\/p>\n\n\n\n

I have been informed that at any time, by contacting the Controller in writing, I have the right to request information about the processing of my personal data, revoke my consent to the processing of personal data, and also demand that any necessary changes shall be made to my personal data for clarification by submitting an application.<\/p>\n\n\n\n

I agree that the Personal Data Controller may entrust the processing of personal data to a third party without prior notice and without my consent.<\/p>\n\n\n\n

This consent is granted taking into account Part 1 of Clause 6 of Art. 5, Paragraphs 2 and 3 of Clause 1 of Art. 16 of the Law dated 07.05.2021 N 99-\u0417 \u201cOn the Protection of Personal Data\u201d and is valid until this consent is revoked.<\/p>\n\n\n\n

I confirm that I have been provided with information about the Personal Data Controller, the composition and content of the collected personal data, as well as information about my rights, as defined by the Law dated 07.05.2021 N 99-\u0417 \u201cOn the Protection of Personal Data\u201d, for the Controller to fulfill the specified purpose.<\/p>\n\n\n\n

I have been informed of the rights related to the processing of my personal data, the mechanism for implementing such rights, as well as the consequences of the consent of the subject of personal data or refusal to give such consent.<\/p>\n\n\n\n
